Starting on Jan. 1, 2015, the reporting system for Professional Review Governing Boards registered with the Division of Professions and Occupations (DPO) is available. DPO is required by statute to collect data in a non-identifiable and aggregate manner; therefore, reporting can only be submitted online and cannot be mailed. Reporting can be submitted starting on Jan. 1, 2015 and will close on March 1, 2015.

Why do I have to submit a report?
Pursuant to section 12-36.5-104.6(2)(b), C.R.S., each Professional Review Governing Board that establishes one or more professional review committees to review the practice of persons licensed under article 36 or licensed under article 38 and granted authority as an advanced practice nurse is required to report annually to the Colorado Medical Board, Colorado Board of Nursing and the Division of Professions and Occupations. Information on Committee actions taking place from Jan. 1, 2014 through Dec. 31, 2014 must be reported. After submission, the annual reporting requirement will be met.

How do I submit a report?
When you log in to your account in the Professional Review Governing Board system after Jan. 1, 2015, you will see an option to submit an Annual Report. Your User ID and Password is what was originally created when your Professional Review Governing Board was registered with DPO. If you have forgotten your password, please use the “Password Finder” feature found on the left side of the Professional Review Governing Board system.

The system can be accessed at https://www.dora.state.co.us/pls/prcweb/PRC.Logon, or by visiting the Professional Review of Health Care Providers website at www.dora.colorado.gov/professions/profreview, click on the left tab “Registration and Reporting” and then log in to your account.

What information will I need?
Please visit the FAQ’s document on our website at www.dora.colorado.gov/professions/profreview to see what information will need to be collected in order to submit your report.

PLEASE NOTE: Only submit information once it is finalized. Once submitted, it is NOT possible for you, or DPO staff to view, edit or delete a submissions or any answers. Please only submit your information once.
