The State Innovation Model (SIM) Practice request for applications (RFA) was released on Sept. 15, 2015. The RFA will remain open until Oct. 26, 2015. The RFA as well as that actual online application can both be found on the new UC Denver Practice Transformation website: www.ucdenver.edu/practicetransformation.

The SIM initiative, a $65 million operating agreement with the federal government, aims to improve the health of Coloradans by providing access to integrated physical and behavioral health care services in coordinated systems, with value-based payment structures, for 80 percent of Colorado residents by 2019.

This Practice RFA is specifically for primary care practices interested in participating in the first SIM cohort scheduled to kick off Feb. 1, 2016. Practices will be notified of their selection on Dec. 25, 2015.

In order to provide those interested with an opportunity to ask questions relating to the RFA, three webinars will be on Sept. 23 (12:15-1:15 p.m.), Oct. 7 (7-8 a.m.) and Oct. 22 (12:15-1:15 p.m.)—all covering the same basic information about the SIM Practice RFA purpose and application process. Get more information about the webinars through this link. Register for any of the webinars directly through this link: SIM Practice RFA Webinar Registration.
