Begin implementing a budget-neutral, value-based payment modifier under the Medicare physician fee schedule. The payment modifier will be applicable to specific physicians and groups of physicians as the HHS Secretary determines appropriate.

• Implementation date: Jan. 1, 2015. By Jan. 1, 2017, the payment modifier will be applicable to all physicians and physician groups.

Reduce Medicare payments to hospital for hospital-acquired conditions

• \Implementation date: Fiscal year 2015

Reduce Medicare physician payments by 1.5 percent for those that don’t participate in PQRI (after 2015 will be 2 percent)

• Implementation date: Jan. 1, 2015

Establish a cost-neutral Medicare Pay for Performance Pilot Program that could be expanded after 2018 depending on success

• Implementation date: Jan. 1, 2016

Establish high-cost (Cadillac) insurance excise tax

• Implementation date: Jan. 1, 2018
