HCPF calls for applications for Accountable Care Collaborative

The Department of Health Care Policy and Financing (HCPF) has announced the opening of the second cohort of the Accountable Care Collaborative (ACC) Chronic Pain Disease Management Program and invites any interested primary care practice to apply.

Free informational webinars will be held on Thursday, Feb. 18, 12 - 1 p.m. MT, and Friday, Feb. 19, 8:30 - 9:30 a.m. MT. Click here to register for Thursday and here for Friday.

The program is modeled after New Mexico’s Project ECHO (Extension for Community Healthcare Outcomes). The first cohort engaged 84 providers from 42 practice sites from every region across the state. The Chronic Pain Management Program leverages cutting-edge telehealth technologies to connect providers to specialists.  Providers consult directly with specialists regarding client cases and learn best practices for treating clients with a variety of chronic pain conditions.  HCPF will sponsor approximately 40 practice sites to serve as telehealth pioneers for Colorado in 2016.
